Job Summary:
The Coordinator I (‘Ohana Conferencing Specialist I) coordinates, engages families, social workers and service providers in preparation for professional, productive, efficient, respectful, compassionate and safe ‘Ohana Conferences and re-conferences. ‘OC Specialist I are responsible for completing all pre-conference tasks ensuring that all key participants will be attending the conference, that the purpose of the conference is clear and understood by all participants, and that the logistical aspects of the conference are planned and delivered. ‘OC Specialist I work in a collaborative environment, with a team of other ‘OC Specialists, Scheduler, Supervisor and Administrative staff. Collaboration as needed with all other programs statewide. Responsible for all steps as described in the ‘OC Specialist manual, leading to the successful scheduling of the ‘Ohana Conference or re-conference within contractual timelines. Responsible for accurate organization and maintenance of the conference case file containing all appropriate and updated documentation, including case notes, Child Welfare Services Safe Family Home Report (CWS SFHR) , Child Protective Services System (CPSS) Intakes, copies of temporary restraining orders (TRO’s) where appropriate and other miscellaneous reports. Responsible for accurate daily case note activity on EPIC database on all ‘Ohana Conference‘s assigned. Upload relevant documents into the database. Accurately document from all key participants, information such as concerns, dates of availability, contact information including fax number, telephone, mailing and e-mail address. Responsible for all logistical aspects of conferences including securing a site, pickup and delivery of packets, working with administrative staff to secure flights and transportation when necessary. Know and practice all current coordination and recording procedures. Responsible for completion of prison interviews for all incarcerated parents and producing a summarized report for the social worker (SW), Facilitator, Administrative staff and case file so that the concerns and thoughts of the incarcerated individual can be shared at the conference. Responsible for appropriate use of all coordinating tools. Responsible for following procedures in the ‘OC Specialist Manual. Verify legal status and timelines of all of the cases and Ohana Conference referrals and re-conference assignments. Timely communicate with and prepare packet for the Facilitator and Recorder, which will contain all information pertinent to the ‘Ohana Conference, within contractual timelines. Provide outreach services as needed regarding but not limited to ‘Ohana Conference in-person information to family and or other key participants who may not be reached otherwise. The coordination case load for an ‘OC Specialist I is 20 to 25 cases with a goal of scheduling 8 to 10 conference per month.
Minimum Requirements:
Must have a Bachelor’s degree in a related field from an accredited institution of higher learning and a minimum of one (1) year related experience and/or training. Must maintain an ethical, responsible and professional attitude and demeanor under all circumstances in person, by phone, e‐mail and correspondence, in order to effectively and efficiently engage and maintain working relationships with families, professionals and other participants who are undergoing complex and crisis situations. Experience working with families whose children have been harmed or threatened with harm and in facilitation and/or mediation, preferred. Culturally relevant training and/or experience specific to the community served, preferred. Team player with a positive attitude and the ability to interact with all levels of the organization. Demonstrated a high degree of professionalism and maturity. Committed to quality and performance improvement. Successful criminal and child abuse and neglect background clearance as required by state regulations for those who work with children and families in the Child Welfare System.
